2 easy steps to book with your first Medical Nutrition Therapy counseling session:
Prior to scheduling an appointment with us, please have the following items available:
Copy of your insurance card information (front and back). Currently, we accept Medicare (Original), Aetna, United Healthcare, Cigna, and Anthem Blue Cross Blue Shield. We are currently working to credential with other insurance providers at this time. We also accept self-pay options with optional payment plans. The insurance card information will help us verify your coverage. See the New Patient page for more details.
You can email or fax your insurance card information (front and back) or ask your provider (Primary Care Provider, MD, or DO) to do so on your behalf.
Our email address is RDnutrition@healthynutrilife.com and our fax number is 626-816-3697.Have your Primary Care Provider (PCP), MD, or DO fill out and sign the Medical Nutrition Therapy (MNT) referral form 24 hours prior to your appointment with us.
You can have your provider fax the form to 626-816-3697 or email the referral sheet to RDnutrition@healthynutrilife.com.
