Important Disclaimer for Booking Appointment!
Prior to scheduling an appointment with us, please have the following items available
Your insurance card information. Currently, we accept Medicare (Original), Aetna, and United Healthcare, and we are working to credential with others at this time. We also accept self-pay options with optional payment plans.
Your PCP’s name and contact information
Have your Primary Care Provider (PCP), MD, or DO fill out and sign the Medical Nutrition Therapy (MNT) referral form 48 hours prior to your appointment with us.
The insurance card information and MNT referral form will help us verify your coverage and enable us to bill your insurance carrier for the reimbursement of the nutrition consultation services. See the New Patient page for more details.
